EFFECTIVE 1 JAN 2026: Annual Player Fees will be required for participation in CFW events. Details will be provided during application processing.

We would first like to take this opportunity to thank you for your interest in the Central Florida Warriors (CFW) Hockey program. On behalf of a grateful nation and state, we would like to thank you for your service and for your sacrifice. Heroes like you make our great country the greatest in the world.

USA Hockey requires that all programs be fully sanctioned with all teams, players, volunteers, coaches, and staff, such programs must be fully registered with USA Hockey. To be eligible, the Player must have a discharge under honorable conditions with the ability to provide official documentation from at least one of the six branches of service (Army, Air Force, Navy, Marine Corps, Coast Guard, Space Force) all reserve and National Guard components are also included. Any exceptions must be approved by the USA Hockey Disabled Hockey Section (submitted to USA Hockey by CFW's VP of Hockey Operations).

You must also meet one of the following criteria for eligibility:

1. Players must be able to provide documentation stating that they currently possess a minimum of 10% service-connected disability rating under honorable conditions issued by the Department of Veterans Affairs.

2. Players who have been awarded a Purple Heart Medal are eligible regardless of their current Department of Veterans Affairs disability rating status, but must have a discharge under honorable conditions. Players must provide a copy of their Department of Defense-issued orders stating that they have been awarded the Purple Heart Medal, along with a DD214 Member 4 document stating their service characterization as Honorable.

Proof of eligibility must be presented for verification PRIOR to team check-in at USA Hockey events. Eligibility documentation will not be accepted on-site at the event. USA Hockey will not keep copies of the presented documentation; it must simply be shown and approved by the USA Hockey Manager of Disabled Hockey and or the USA Hockey Warrior Hockey Representative.

As part of registration, you must complete the following steps:

1.    Complete the Become A Warrior registration form (left button at top of page)
2.    Read the Central Florida Warriors Player Handbook (right button at top of page)

Once the above steps are complete, you will receive an automated email directing you to send several supporting documents to austin@cfwarriorshockey.org. Documents needed are:

  • USA Hockey Registration Confirmation Page for the current year as a PDF

  • Completed USA Hockey Consent to Treat form as a PDF

  • Proof of service and disability rating - can be separate documents listed below:

    • Proof of disability ratings as a PDF (Choose one):

      • Copy of Purple Heart documentation and DD214 Member 4

      • VA Benefit Summary and Service Verification Letter that includes Character of Service as “Honorable”

For safety reasons, all players must possess basic Adult League equivalent hockey skills and skating ability (assessed by Central Florida Warriors staff) and full ice hockey equipment. 

Once your registration and additional documents have been received and reviewed, you will be contacted with further instructions.
